The contact owns a 2025 Chevrolet Blazer. The contact stated that the vehicle experienced repeated phantom braking while driving at various speeds. The contact stated that the automatic emergency braking system activated erroneously. The automatic emergency braking system was deactivated to prevent the failure; however, after a software update at the dealer during a radio repair, the feature automatically activated. After the update, the phantom braking reoccurred on several occasions. The contact had successfully disabled the feature again but remained concerned that the system might activate unexpectedly while turned off. The failure mileage was approximately 10,000.

  • Service brakes, forward collision avoidanceFiled Sep 14, 2026

I am concerned about a significant safety issue with my 2025 Chevrolet Blazer EV. On at least four separate occasions, the vehicle has unexpectedly applied extremely hard Rear Automatic Braking while I was backing up, despite there being no object, vehicle, person, or obstacle behind the vehicle. The braking is so abrupt and forceful that it feels as though the vehicle has collided with something. Because the driver is typically looking over their shoulder while reversing, the sudden stop causes a violent jolt. This has resulted in neck pain and headaches for me and my son on multiple occasions. One of these incidents also occurred while my son was driving the vehicle. We discussed the event afterward and both experienced the same sudden, aggressive braking with nothing present behind the vehicle. The system appears to be activating falsely. This is a serious safety concern. The unexpected braking could potentially cause injury to the driver or passengers and may also create a hazardous situation if another vehicle is following behind. I am requesting that this issue be investigated and repaired as soon as possible.

While backing into parking at a Humble Cow ice cream place. My 2025 Chevy Blazer EV AWD slammed to a stop. I was going 4 mph my spouses head hit the head rest and caused severe pain and stress. We had to go to the emergency room for treatment. This has happened 6 other times 3 since having the required updates and replacement of modules that took several months to complete. She was given medication and lytocain patches for the trauma of her neck and head. She is now afraid to drive in my vehicle. I looked up the situation and found on Reddit and the internet that this is a known issue with Chevy and several other vehicles relating to their braking and eboost system. One work around is to turn off all of the sensors from the settings menu to include parking. It also said to not use the one pedal driving. This is unattainable for a brand new car that I paid Cash 45-50,000 for and not to be able to use it as intended. Please look into this. I will be filing another complaint with the dealership on 14 September 2026. Something must be done.

The onstar module failed. Also controlling the pedestrian px Proximity sensor and crosstraffic sensor. When driving slowly waiting a parking lot the vehicle locked up on the brakes with the warning message pedestrian alert. The area was clear. No pedestrians were in the vicinity This would be the 3rd incident 1st was cross traffic already braking while entering a light only at a speed of 15 mph. 2nd was the cross traffic and 3rd was the pedestrian alert.. We discovered the onstar module contrs all of these functions. Contacting the dealer theu stated the module need to be replaced but there is not an ETA on the part arrival as this is a common issue with all vehicles that contain. This module. We are scared it will happen if a driver pulls in front when in traffic or put us in a precarious even deadly situation

      1. Vehicle has a horrible squeaking sound coming from the brake pedal area. First dealer diagnosed it as the master cylinder needing replacement. Noise did not go away and GM said that it is normal. The noise is not normal and has never been on any other car I have owned. 2. When reversing, the car sometimes abruptly slams on the brakes as if it is avoiding a collision even though no collision is close to happening. GM refuses to fix the problem.

At a low speed, between 10 and 15 mph, the forward collision system automatically triggered and brought the car to an immediate stop. While we were lucky that no car was following behind us, the passenger and I experienced significant whiplash and lasting back pain as a result of this. The vehicle has not been inspected by anyone, and not been reproduced. The forward collision system flashed (red warning light, alert).

    • Electrical system, service brakes, forward collision avoidanceFiled Nov 13, 2025

Multiple safety-critical malfunctions have occurred on this vehicle involving unintended braking, unintended acceleration, and false activation of driver-assist systems. These events did not happen at the same time and occurred under normal driving conditions. At various points, the vehicle has suddenly applied the brakes without driver input, including hard braking events that felt like an automatic emergency braking activation when no hazard was present. In separate incidents, the parking brake has engaged while the vehicle was in motion, causing abrupt deceleration. There have also been instances of unintended acceleration where the vehicle surged forward briefly without additional throttle input. These malfunctions appear to be related to electrical or ADAS system faults affecting braking logic and propulsion control. No warning lamps or messages reliably appeared before these events. The issues pose a serious risk to the safety of the driver and others because the vehicle’s behavior is unpredictable and can interfere with safe control. The dealership was unable to reproduce the conditions during inspection, but the incidents have occurred multiple times in real-world use.

Rain is getting inside and soaking into the dashboard causing the electrical system on screens to blink in and out Finally my brake harness is still on recall and my car has been shifting into park which could cause severe problems if I am on the highway..

This is a report of defective software that could increase the chances of an accident, injury and or death. This vehicle features a built-in garage door remote which is utilized exclusively through the touch screen. However, when the vehicle is in reverse the backup cameras restrict access to the needed menu. After backing up and putting the car in drive there is a delay before the back up camera turns off. This creates a situation in which the driver must wait in the middle of the road after placing the car in drive before being able to close the garage door. In seeking solutions from online forums, some drivers have reported their Blazer EVs allow the button to be accessed while others report they do not, so I speculated perhaps I missed a software update, but per the car's OTA system, the vehicle's software is current as of the time of this submission. I have recorded a video that I am happy to share , however it exceeds the size limit allowed for uploads in this report.

The dealer was contacted and diagnosed that the windshield was not installed properly and needed to be replaced. The contact stated that while raining, water leaked into the vehicle through the windshield onto the dashboard. The contact stated that the seal had detached on the passenger’s and driver’s sides of the vehicle. The contact stated that while driving at an undisclosed speed, the vehicle shifted into park(P). The vehicle had to be turned off and restarted to shift out of park(P). The failure had occurred while driving, idling, and while parked. There were no warning lights illuminated. The dealer also discovered that the hood of the vehicle and the windshield were not flush, causing a slight detachment. The vehicle was not repaired. The contact was informed that the vehicle could not be repaired, and a buyback was offered for the vehicle. Additionally, while driving at an undisclosed speed, the Forward Collision Avoidance system erroneously activated, causing the vehicle to abruptly stop in the middle of the road. The vehicle was taken to the local dealer, who reprogrammed the Forward Collision Avoidance sensors, but the failure persisted. Additionally, the charging port had malfunctioned and became inoperable. The charging port was replaced, and the vehicle was repaired. The manufacturer was notified of the failure. The motorized charge port door no longer opens. There is no manual operation rendering the vehicle unable to charge. This presents a potential hazardous situation if the error occurred during a road trip where there are greater distances between charging stations and services. GM customer service and OnStar could not resolve the issue when contacted.
