Vehicle: 2025 Chevrolet Traverse LT FWD VIN: [XXX] Purchase Date: XXX Incident Date: XXX Mileage: 35,975 INCIDENT DESCRIPTION: On Fr XXX, just seven days after purchasing this vehicle from Enterprise Car Sales, I started my 2025 Chevrolet Traverse and was immediately presented with multiple serious braking and vehicle safety-system warnings. The primary warning stated: "BRAKE ASSIST FAILURE. PRESS HARD TO BRAKE. SPEED LIMITED." Additional warnings included: - Service ESC (Electronic Stability Control) - ABS warning light illuminated - Traction/stability control warning - Forward Collision System Unavailable - Red BRAKE warning illuminated - Check Engine light illuminated These warnings appeared during the same startup event while the vehicle was stationary. SAFETY CONCERNS: I am extremely concerned that a vehicle with fewer than 36,000 miles is reporting a potential failure of its power-assisted braking system, along with simultaneous malfunctions involving ABS, stability control, and forward collision protection. The dashboard specifically instructed me to press harder to brake and indicated that vehicle speed was limited. This raises serious concerns about braking performance and the ability to safely stop the vehicle, particularly during an emergency. I am aware of NHTSA investigation EA26006 concerning potential electronic braking-system failures in certain General Motors vehicles, including the Chevrolet Traverse. The symptoms reported in that investigation appear similar to the warnings displayed by my vehicle. The underlying cause has not yet been confirmed. MANUFACTURER NOTIFICATION: I formally reported this incident to Chevrolet/General Motors. GM Case Number: XXX Photographs documenting the warnings and mileage are available. REQUEST: I request that NHTSA document this incident as a potential vehicle safety defect and evaluate whether it relates to existing complaints or investigations INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Driver side windshield wiper does not clear window, blades have been replaced and still not fixed. Appears to be a wiper arm issue. Rear camera blips out, goes black or shows half a screen during use. Happens almost daily and has caused me to nearly hit cars and or people. Front tire sensor will not recalibrate. Brakes make horrible squeaking noises- Dealer advised this is a known issue with the type of brakes installed on these cars and nothing can be done. Taking car in for service for all these issues.

Two issues have been occurring with my car amongst many others, but these are the most recent my vehicle has all seatbelt sensors go off despite no one else being in the vehicle, but me the driver my sensor does not go off. It will not stop beeping or screaming at me until I have to pull over turn off my car turn it back on and essentially reset the system. The other thing my car intermittently when I shifted into park at a full stop for example like in my driveway my foot is fully pressed on the brake, and my car will completely shut off and jerk the car forward and backwards when I shift into park, the engine shuts off but the car was never turned off just shifted in to park. and then resets my entire car all the electrical panels, the dashboard everything goes off-line, then comes back online and I get a message that says to reset driver window, lower window all the way down and raise it back up to which I later found that this meant the entire car program was just reset and started over… This is really frustrating and I fear of something like this happening while I’m actively driving It is not letting me upload the video

The car starts it self and then shuts off all by it self. It has done this twice that we know of. Its impossible to do this even if your keys are in your pocket as you have to hit 3 different buttons to do this. I am not the only one having this issue as there are people complaining on FB that their car does the same thing.

Sudden slamming on the brake automatically when in reverse and there is nothing around the car to cause this. Has happened many times The car will alert that something is too close when turning right by there is nothing there. It is intermittent. When you start the car it the motor reves up very high and will stay there until you push the gas and then it quiets down. This is usually the first start of the day. High and rough idle Super hard intermittent transmission shift or down shifting while normal driving. It jerks really hard and sometime it’s so hard it throws you forward like the brake is being applied. Symbol intermittently shows up on dash. Then goes away after the car has been turned off and restarted. Brakes squeal bad. Acceleration reduction message then check engine light came on 8/30. Roaring loud noise before the light came on. Put 1 gallon coolant in it and light went off. Now runs and idles high.

    • EngineFiled Aug 24, 2026

Vehicle had large puddle of antifreeze under drivers front of vehicle. Antifreeze reservoir was completely empty upon opening the hood. There is an active drip of antifreeze under the car as displayed by placing a clean piece of cardboard under the vehicle. There were no indicator lights displaying a problem on the dashboard. Upon using an endoscopic camera, there is an active leak at the hose on the radiator. If I had not noticed the active leak, I would have driven the vehicle and overheated the engine with the possibility of seizing the engine. Vehicle is in service currently at Superior Chevrolet. There is a known issue under Special Bulletin N262546590 for the seal at the quick connect inlet on the radiator.

This started happening about two weeks ago. My check engine light came on and my OBD scanner said a NOX sensor code showed. I told the dealership and I guess they cleared the code and it hasn’t reappeared but the reduced acceleration has maintained. It was [XXX] I had just go of work and needed gas. While traveling to the gas station I got notification that my acceleration was being reduced and it was very noticeable. After arriving and pumping gas, I started the car and got 3 additional messages. The car aside my brake assist and ESC needed service, my ABS system was off, and my forward collision system was off. While driving, I noticed that the brakes were still working but acceleration was still being reduced. This is an everyday use car and I have a 6 children who I don’t feel are safe anymore in the car. Not being able to utilize the brakes and the reduced acceleration is very concerning especially when in traffic. I have a little over 27500 miles on the car. It just came from the dealership to get the telematics module replaced last week because it went out about a month ago. I’ve only had this car a year and it seems like the issues are becoming more common everyday. I will be taking care back to dealership tomorrow. This has resulted in no ABS or safety features. Called dealer and can't look at car for over a month. There was no mention about not driving. If the wife needs ABS or front radar, she will not have it resulting in a more serious accident.

    • Visibility/wiperFiled Jul 13, 2026

While driving on vacation approximately five hours away from home with my family, I suddenly heard a loud popping sound followed immediately by increased outside wind noise coming from the panoramic moonroof. Based on the sound, I suspected the moonroof glass had shattered. Because my family was in the vehicle, I intentionally did not open the interior sunshade, as I was concerned that broken glass could fall into the cabin and seriously injure my passengers. After safely pulling over, I inspected the roof from the outside and confirmed that the panoramic moonroof glass had completely shattered. During the drive back, the shattered glass panel completely detached from the vehicle and flew off. Fortunately, it landed in the grass and did not strike another vehicle or person, which could have resulted in a serious accident or injuries. Because this occurred over the weekend, I had no immediate access to repairs. To safely transport my family the five-hour drive back home, I had to temporarily cover the opening with a trash bag and tape to protect the vehicle’s interior and reduce exposure to the elements. After returning home, I contacted the dealership, provided photographs of the damage, and explained the circumstances. I was informed that the failure would not be covered under the vehicle’s warranty and that I would be responsible for the repair costs. I am extremely concerned that the moonroof shattered without any apparent impact from road debris or object. Had the interior sunshade been open at the time, pieces of broken glass could have fallen into the passenger compartment, potentially causing serious injuries to my family, including eye injuries or cuts. Additionally, when the glass panel detached from the vehicle, it could have struck another motorist or pedestrian, creating a significant safety hazard. I believe this incident represents a serious safety concern and request that this matter be thoroughly investigated and reconsidered for warranty coverage.

    • Back over prevention, forward collision avoidanceFiled Jul 2, 2026

Approx 5/19/26 Forward Collision Detection check light randomly came on when driving home and showed notifications showing "Service ESC" "Service Traction Control" then started showing notifications for the collision sensors unavailable and continuously flickering them across the dash. The Fwd collision symbol would come on and off randomly. Finally was able to take it to the dealership on 6/8/26. Dealership found 6 codes and updated the image processing module. The Fwd Collision symbol is back on and off, notifications are flickering across the dash more frequently and now the check engine light is constantly on as of 6/14/26. Car is currently back at the dealership. Pictures and videos were taken of the issue happening to provide extra proof and given to the techs. These sensors failing at any time could cause the vehicle to randomly brake, not brake, and possibly not register a potential accident and avoid it. This vehicle is our daily driver and is the vehicle my son's car seat is in so it's not just my safety, it's the safety of my wife and baby. It's the safety of anyone else who has this type of vehicle with these issues. Understandably it's always the drivers responsibility to pay attention to the road and surroundings but there's nothing the driver can do if the vehicle randomly shuts off or stops or slams on the brakes or another type of malfunction while out on the road. This vehicle has been in and out of the dealership 5 times now for three different things, every time it's picked up, the problem is still there or comes back within days. It's a pain moving all the baby gear back and forth, on top of the rentals we have been given have been so small, you can't even fit a stroller and groceries per-say in the trunk.

Our car one day showed several illuminated dash lights regarding brake system, along with several pop up warnings. We took it to a Chevrolet dealer for service. They’ve had it for 54 days. They’ve confirmed widespread brake system and electrical system failure, but even after escalating to corporate tech assistance, there is no progress nor resolution available. We were told they have at least a dozen others with this same issue. It is wildly inappropriate that there has not been a recall issued as this is a serious safety defect. We demand a buyback from GM for putting this death trap on the market knowing they had these issues with 2024 models but continuing to roll 2025 and 2026 models with the same issues.

I purchased a brand-new 2025 Chevrolet Traverse and have been dealing with a loud brake squeal that makes the vehicle sound like it has worn-out brakes. I’ve taken it to two different Chevrolet dealerships, and instead of fixing the issue, I’ve repeatedly been told that this is a common problem with the 2025 Traverse and that many owners are experiencing the same thing. I’ve been given multiple explanations, including weather conditions and the vehicle needing to warm up, but no actual solution. I was even told there is currently no fix available through GM and that replacing the brake pads will not resolve the problem. What is most frustrating is that everyone seems to acknowledge the issue while also telling customers to simply live with it. This is a brand-new vehicle, and customers should not be expected to accept a known brake defect as normal. Very disappointed with both the dealership experience and GM’s response to what appears to be a widespread issue.

NHTSA Defect Report (Initial Portal Filing) Manufacturer Target: General Motors (GM) Filer: [XXX] Classification: Mechanical Defect & Administrative Evasion Narrative for Submission I am filing this formal defect report regarding a catastrophic, systemic safety failure present in the 2025 Chevrolet Traverse architecture (VIN: [XXX]), manufactured by General Motors. The vehicle was originally purchased at AutoNation Timonium. THE SAFETY DEFECT AND TELEMETRY PROOF On March 18, 2026, the vehicle suffered a total failure of the transmission and drivetrain systems. It was immediately surrendered to Preston Chevrolet, where it remained completely out of service for 34 consecutive days. GM's own OnStar Diagnostic Telemetry automatically dispatched critical fault alerts while the vehicle was stranded, explicitly logging the following irrefutable failure codes: DTC U1962 (PCM - Engine and Transmission System): Warning stated: "System transferring power to the drive wheels not performing as expected." This indicates a catastrophic decoupling or failure to transfer motive power. If this failure occurs at highway speeds, it results in a total loss of propulsion, creating an extreme rear-end collision hazard. DTC U0452 (VIU - OnStar System): A simultaneous failure in the Wireless Communication and Safety system, meaning that when the drivetrain catastrophically fails, the vehicle's automated emergency SOS and crash-reporting telemetry is actively blocked or compromised. ELEVATION TO ADMINISTRATIVE EVASION General Motors and its authorized dealer network were provided with formal written notice of this defect. The 30-day statutory right to cure window fully expired without any mechanical resolution. Rather than deploying engineering solutions to rectify the severe safety defects they released into the public domain, GM has instead deployed aggressive legal maneuvers designed to evade accountability. • Service brakesFiled May 6, 2026

At approximately 30,000 miles, upon starting my 2025 Chevrolet Traverse LT, the brake pedal traveled completely to the floorboards with zero resistance. Simultaneously, the 'Service Brake Assist' warning message appeared. This loss of hydraulic pressure occurred while stationary. This follows a dealership visit one week prior for a 'glitchy' check engine light that was dismissed by service staff. The vehicle is being towed via flatbed to the dealership as it is unsafe to operate. Additionally, the vehicle has exhibited abnormally 'touchy' brake response and a persistent high-pitched squeal from the front assembly since purchase. These symptoms suggest a failure of the e-Boost braking system and/or internal master cylinder seals, consistent with Service Update N242482170. This defect poses an immediate risk of total brake failure during operation.

We drove the Traverse 55 miles to a baseball game. When we got in the car to return home several warning lights appeared on the instrument display and there was an audible warning sound. The message “Service ESC” and “Brake assist failure” appeared on the instrument display panel. We couldn’t find a dealership that was open on Saturday so we cautiously drove 55 miles home. The brake pedal was hard to push and had to be pushed all the way to the floor to stop the vehicle. We could not drive faster than 62 MPH on the way home and I had to start braking early in order to be able to come to a complete stop at stop lights. Cruise control isn’t working either and the audible alarm sounds off every couple of minutes. We took the car to our local dealership this morning.

(Service ESC, Service Brake Assist, Cruise Control Inoperative) Ed Morse Chevrolet and Brad Deery Motors have both confirmed the brake cyclinder being defective. Ed Morse advised not ordering the same brake cylinder as they are known issues from factory, they advised me to wait for the recall or a re-engineered part to be produced by GM. Brad Deery Motors knew nothing about this and they are currently putting the OEM master brake cylinder back on the car. I believe they are putting a known defective component back on my brand-new vehicle. There is currently a law suit in Pennsylvania. • Service brakes, forward collision avoidanceFiled Mar 14, 2026

The first component to fail was the brake booster module which controls the ABS, forward collision, adaptive braking, adaptive cruise, traction control system, etc. The malfunction started when the forward collision assist activated to prevent me from hitting a deer in the road. We avoided hitting the deer but all of those lights came on immediately. We contact Friendship Chevrolet the dealer we bought the car from had us bring it in to be checked. They confirmed the defective Module and said that it was a reported issue that GM had it's engineers looking at. They said they would have to wait for the part to arrive, advised that we could continue to drive the car even though there was a major malfunction with the critical braking systems. did not offer any type of loaner to ensure my family's safety. We had to wait for more than 30 days for them to receive the part and complete the repair still driving this unsafe vehicle because we had no other choice considering how high the MSRP & payment is on the vehicle. Once the repair was finally completed we immediately began to notice that the brakes were consistently screeching when in slow moving traffic like car rider pick up line. I reported the issue to the dealership and they gave me the answer that it was due to a more abrasive brake pad being used due to it being a passenger vehicle and it's weight. We continued to deal with the squeaking for months then we noticed a coolant leak. Dealership again said had to wait for part. After they did not contact me back I contacted GM and opened a case. When they got it in I also had them change the brakes out of my own pocket. Now the brakes squeak anytime pressure is applied and when coming to a complete stop they grind so loud you can hear and feel them digging in. GM still says nothing is wrong and brakes are a maintenance charge and are not covered for any reason. I feel the vehicle is still extremely unsafe and my family is at risk of possible crash.

Turned on vehicle on 3/2/2026 and received a message stating "Anti-lock break system failure. Drive slow and stop cautiously." I test drove my vehicle in my local neighborhood and discovered that my brakes were only functioning at about 20% and would only respond if the brake petal was pressed all the way down. My car was towed to the local dealership to be assessed and was determined to need Reservoir 4.651, Cylinder 4.650, and Fluid 8.800 when it was picked up on 3/6/2026. Under further review of this report I came to discover a class action lawsuit for this vehicle make and model relating to cylinder part #85762161. I am unsure if my original part was that part number, but my experience closely mirrors other's that have been reported for the same issue and part repair. The part that was also replaced in my vehicle after the repair was part #85762161. Some individuals in this lawsuit have reported 100% brake failure. I am fearful to continue driving the vehicle knowing that there could still be ongoing issues with the part that directly relates to the brakes.

I was driving at night on a dark road My truck showed “ Brake assist failure. Press hard to brake. Speed limited.” I tried to step on the brakes & the truck didn’t stop. I was able to eventually turn the truck off & when I restarted it was fine. Luckily it wasn’t a busy street or that I didn’t have my 3 kids with me because it could’ve ended differently. I set an appointment at my dealership & when I went to start my truck again 1. It was dead and 2 when I jumped it, that same message popped up again & my truck was towed to the dealership on 2.25 & I haven’t heard back from the dealership since with an update.

    • Electrical system, service brakes, engineFiled Feb 19, 2026

Initially, the engine would intermittently fail to start and at times would not start at all. I took the vehicle to a certified Chevrolet service center, where the battery was tested and found to be functioning properly. They later concluded that a faulty front-view camera was disrupting the electrical system and ordered a replacement (my first visit was Friday, 9/26/25). I took the car home for the weekend due to family obligations. On Monday, 9/29, while driving back to the service center, the engine suddenly shut off on a local road. The brakes went completely to the floor for 3–5 seconds before the engine restarted—just before I nearly collided with another vehicle at a red light. I reported this immediately, and the dealership kept the car for further testing. On Thursday, 10/2, they informed me they found no additional issues and believed the camera was the root cause. However, the problem persisted. On Sunday, 10/5, while stopped at a red light at Hawthorn St. and Memorial Dr., the engine shut off again as I began to accelerate. The brakes sank to the floor, and I had no steering control for approximately 5–7 seconds before the vehicle restarted. My wife and I were extremely shaken, especially with our children in the car. On Monday, 10/6, we contacted roadside assistance to tow the vehicle back to the service center. After additional testing, we were informed that the battery had failed this time and was replaced. However, replacing the battery does not reassure us that the engine and braking system will not fail again while driving, or the brake failure would happen again when the battery is low - which poses a serious safety risk to our family. When my wife called GM to report this issue, all they offered was 200 points worth of rewards....We originally bought this car for safety, but not this has been compromised and we no longer feel safe. Thank you for your attention to this matter. I am happy to provide any additional information if needed.

Purchased the traverse January 10, 2026 with almost 26k miles on it. Approximately one month later, only 1200 miles or so into owning it, the shark fin antenna cover came off and is missing. It’s not the entire antenna that is hooked into the car, it’s only the plastic cover that covers up the electronics. This plastic piece randomly flying off the car put other motorists at risk, due to the plastic cover potentially striking another vehicles windshield distracting them from keeping their eyes on the roadway, striking another motorists vehicle causing damage to said vehicle, or even striking a person walking down the street potentially resulting in physical harm! I have talked with GM dealerships about this issues along with reading that numerous people have had the same issue, and they have yet to fix the problem.

While driving, ABS, traction control, ESC, forward collision, and AWD alerts were indicated by the car. At the time it had 10 months in service and approximately 12,800 miles accumulated. After 17 days at the dealers service department and a direct call to GM the brake master cylinder and associated components were replaced. I've seen several reports of this generation of Traverse having this same issue for other owners at low mileage. I want to file this report to ensure that NHSTA is aware of this serious safety concern that could result in loss of braking power. It appears to be widespread and not addressed by any recall and dating back to the 2024 model year change to the most recent generation.

I turned my vehicle on and my brakes, ABS, traction control and anti collision lights all came on. Upon research this is a known problem in 2025 and a recall in 2024. It did in fact end up being the master cylinder and brake system that faulted
