On August 23, 2026, the electronic power steering and braking systems on a 2025 Honda Accord Hybrid completely failed and locked up simultaneously while traveling at highway speeds. This caused an immediate and severe safety risk, resulting in a near-collision on a busy freeway with three occupants inside the vehicle. The vehicle is currently available for inspection upon request. Prior to and during the failure, widespread dashboard warning lights illuminated simultaneously, including the Collision Mitigation Braking System, Lane Keeping Assist System, Road Departure Mitigation, and Low Speed Braking Control. Additionally, 11 malfunction alerts were triggered via the HondaLink app. The vehicle was inspected by an authorized dealer from August 24 to September 11, 2026. As documented in the attached Honda dealer service report, technicians initially noted low voltage codes causing multiple system failures. However, instead of identifying and repairing the root cause of the voltage drop, the dealer merely cleared the codes, test-drove the vehicle, and closed the repair order stating they "could not duplicate concern." Furthermore, the attached text message conversation with the service advisor proves the service department explicitly admitted in writing that the software is up to date and that the failure could happen again if the vehicle encounters the same error. The vehicle has not been inspected by a manufacturer field engineer, police, or insurance representatives. American Honda Corporate refused requests to dispatch a field engineer and closed the case file. Note: Full video evidence of the dashboard warnings and loss of vehicle control during the incident exists, but cannot be attached here due to file format limitations. The manufacturer and dealer have returned this vehicle without identifying or repairing the underlying defect, leaving it highly susceptible to another catastrophic loss of steering and braking at highway speeds.

AIR BAGS,SERVICE BRAKES

The contact's son owns a 2025 Honda Accord. The owner stated that while coming to a stop, the brake pedal was depressed, but the vehicle failed to stop. No warning light was illuminated. The owner fully lifted the foot from the brake pedal and forcefully depressed the brake pedal while attempting to stop the vehicle. The vehicle failed to stop, causing the contact's son to rear-end a semi-truck. The contact's son stated that the vehicle powered off after the crash. The contact's son also stated that the air bags did not deploy during the crash. The vehicle was taken to the dealer, but the failure was not diagnosed or repaired. The dealer informed the contact that the failure could be duplicated. The manufacturer was made aware of the failure by telephone, and the contact was corresponding with a case manager via email. A case was opened. The failure mileage was approximately 14,000.

STEERING

Recurring steering/front-end clicking/tapping noise occurs when turning the steering wheel left or right, including while the vehicle is stationary and at low speed. I first noticed the steering clicking noise at approximately 1,500 miles but initially overlooked it. The issue was previously confirmed by an authorized Honda dealer. In September 2025, the technician documented that the noise was easily duplicated while driving or stationary, used diagnostic equipment to locate it, identified the clicking/tapping as coming from the EPS steering rack assembly, and replaced the EPS rack under warranty. A later service record stated that the steering clicking problem was fixed. The same/similar clicking noise later returned. In August 2026, the dealer again documented the complaint but stated that the noise was a “normal characteristic” of the model/year rather than identifying and permanently correcting the cause. I have spent many personal hours testing, documenting, and comparing the vehicle with other similar Honda Accords. I obtained multiple comparison videos showing comparable vehicles tested under similar stationary steering conditions that do not produce the repeated clicking noise present in my vehicle. I provided this evidence and the service history to American Honda Corporate. American Honda also published a March 20, 2026 Tech Line Summary, “Clicking Sound While Turning the Steering Wheel,” applicable to 2023 and later Accord models, describing a clicking/light ticking/binding condition involving the steering gearbox/rack area. Because this recurring condition involves the steering system, I am concerned about safe vehicle operation if the underlying cause is not properly identified and repaired. My family is uncomfortable riding in the vehicle. The condition remains unresolved, and the vehicle is available for inspection. Supporting service records and additional documentation are being uploaded.

The panel computer says "the system updates failed, please see dealer", however the dealership has assisted with the panel and advise the problem needs to remedied by Honda directly. Having the issue presnt can infer with the function and safety of the vehicle's computing system

The vehicle was being driven in the rain on August 27, 2026, at approximately 8:30pm, when the power steering, power breaking, and all other electronic modules needed to safely operate the vehicle were loss. NHTSA Recall 24V-744 currently covers 2023-2025 Civics and CV-Rs but not Accords.

- The steering system appears to be malfunctioning. The steering wheel becomes sticky or feels like it catches while turning, and there is a clunking and grinding noise when the steering wheel is turned. The vehicle and affected components are available for inspection upon request. - The steering issue creates a potential safety risk because the steering wheel may not turn smoothly or respond as expected. The sticking sensation and grinding/clunking noises could affect my ability to safely control the vehicle, especially while turning, parking, or avoiding a hazard. - Not yet. The problem has been experienced while operating the vehicle and needs to be inspected and diagnosed by a qualified service center. -Not yet.The vehicle has not yet been inspected regarding this specific issue. -There were no warning lamps or messages displayed, if applicable. The symptoms include sticky or catching steering, along with clunking and grinding noises when turning. The symptoms first appeared recently and occur while turning the steering wheel.

TIRES

The contact owns a 2025 Honda Accord. The contact received notification of NHTSA Campaign Number: 26V366000 (Tires); however, the part to complete the recall repair was not yet available. The dealer was contacted and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact had not experienced a failure.

FORWARD COLLISION AVOIDANCE

Automatic braking has engaged at least 3 times in the last year when there was no car nearby or obstacle in the road. This has happened on city streets at speeds of approx 30-45mph. If this sudden braking happened at freeway speeds it's highly likely I would be rear ended. I have not taken it to dealer as I expect that they will say what they typically do ...nothing is wrong. Granted it is difficult to reproduce the phantom braking for them to observe the issue. The alarm sounded and the large red "Brake" light came on on the dash. 8.12.26 incident was in a lane adjacent to traffic cones but I was nowhere near hitting them.

The vehicle's infotainment system displays an OTA software update error message on every startup. Honda dealerships are unable to address this issue without further intervention and software updates from Honda corporate. Dealer has stated that this issue may interfere with reliable over-the-air software updates, including any potential safety-related updates delivered through this OTA system.
