  • Fuel/propulsion systemFiled Oct 7, 2026

What component or system failed or malfunctioned, and is it available for inspection upon request? The plastic fuel tank failed. It cracked/split open and is actively leaking gasoline. The failure is at or near the filler inlet connection where the filler pipe attaches to the tank. The tank is still installed on the vehicle and is available for inspection upon request at Lakeland Chrysler Dodge Jeep Ram (or can be made available wherever the vehicle is located). How was your safety or the safety of others put at risk? Gasoline is leaking from the tank onto the ground. This creates a serious fire and explosion hazard, especially near the exhaust system. The vehicle cannot be safely driven and was immediately taken out of service. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The vehicle was inspected and diagnosed by Lakeland Chrysler Dodge Jeep Ram on or about October 6, 2026. They confirmed the fuel tank is cracked and leaking and issued a written estimate for complete fuel tank replacement (parts and labor approximately $2,525 for the tank alone). The replacement tank is currently on backorder with an estimated two-week wait. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected and diagnosed by the Ram dealer (Lakeland Chrysler Dodge Jeep Ram). It has not yet been inspected by the manufacturer, police, or insurance representatives. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. There were no warning lamps, dashboard messages or check-engine lights. We started smelling gas and it was leaving a puddle when parked. There was enough on the ground to catch fire

  • Service brakes, hydraulic, service brakesFiled Sep 28, 2026

This vehicle was a rental RV from Roadsurfer in [XXX]. Electronic issues caused a fault in the ABS brake system that eventually damage the front axle assembly, which was replaced during our vacation. The electronic issue was not addressed so the ABS brake problem continued throughout the trip, causing major safety issues as the brakes kept trying to set while driving at highway speeds. After the repair was completed, we went to an auto repair store to see if we could see codes that were being thrown, and there were multiple codes already present for this issue when we rented the RV. We also discovered an airbag sensor signal that had not been addressed. we then went to a dealer for this model Van and got an additional list of airbag and driver seat sensors that had active codes. The ABS brakes continued to have issues up to every 20 minutes during the entire six day vacation. The rental company was called multiple times and a claim was filed. These issues began with an 80 miles of picking up the RV on XXX and continued until we returned it on XXX. I inquired multiple times throughout the trip if we could return it have a new Van brought to us exchange it, and no options were given. Even issues that are noted in the rental contract are not addressed as repairs are not being done as documented through conversations on social media. • Steering, electrical system, lane departureFiled Aug 13, 2026

Sometimes while driving I experienced my powered steering cut out while taking a ramp on/off of the highway, and sometimes when i was trying to park and back up into a parking space while steering at the same time. When the power steering cut out I got lights for "Service Power Steering", "Service ESC", and "Service Hill Assist" all at the same time, and once i got a power steering light. I looked up the issue and found that there is an active preliminary investigation going on for the same issue on the same vehicle, the investigation coded PE26002. Once I handed the vehicle over to the dealer, they found that the power steering had an open circuit on the steering gear. Once they took the connector off they found that the connector was corroded and the wires were burned on the steering gear and main harness side. I had to have them replace both the steering rack, and the main harness.

Steering Rack and Pinion failure due to corroded electrical connection plug. This occurred in april, 2025, when the van had less than 15,000 miles on it. Took the van to Blue Ridge Chrysler Dodge dealership in Abingdon Va .for Warranty repair. Plug was taken off another vehicle to put on mine aaaand dielectric grease added to hopefully keep the replacement plug from corroding too. Location of plug makes it susceptible to water infiltration, a definite design flaw that is likely to cause trouble again in the future.

      Incident Jul 9, 2026

    • Steering, electrical systemFiled Jul 17, 2026

While driving, a “Service ESC” warning message appeared and the vehicle suddenly lost power steering assist, causing the steering to become extremely difficult and creating a serious safety risk while the vehicle was in motion. The vehicle was taken to a service center, where technicians found multiple loss-of-communication codes involving the electronic power steering system. Further inspection found the main electronic power steering connector full of water and corroded. The technician documented that water intrusion caused excessive damage to the electronic power steering terminals at the rack-and-pinion assembly. The water intrusion appears consistent with a failure or inadequacy of the sealing intended to protect the electronic power steering connection from moisture. Due to the extent of the corrosion and damage, the affected wiring harnesses and rack-and-pinion assembly had to be replaced, followed by a wheel alignment. This failure resulted in a sudden loss of power steering assist while the vehicle was being driven.

    • EngineFiled Jul 14, 2026

While operating a 2023 Ram ProMaster 2500 (3.6L V6) for normal commercial use at approximately 39,129 miles, the coolant began bubbling and engine temperature spiked suddenly while driving. The vehicle was immediately parked and towed to an authorized Chrysler Jeep Dodge Ram dealership as it was completely undrivable. This created a serious safety risk as the vehicle lost drivability suddenly on public roads during active commercial operation. Upon teardown the authorized dealer documented: coolant leaking into cylinders 3 and 4, coolant coming out of exhaust, stretched engine bolts, warped block and head, and internal coolant leak causing overheat. No external leaks from the cooling system were found. The internal component failures caused the overheating not the reverse. The vehicle was still within its active powertrain warranty at time of failure. Stellantis denied the warranty claim under TSB D-23-20 claiming an undetermined overheat caused the engine failure directly contradicting their own dealer's findings which show zero external cause. I have paid $9,000 out of pocket for repairs that should be covered under active powertrain warranty on a 3-year-old vehicle under 40,000 miles. I am requesting NHTSA investigate this failure pattern in the 3.6L V6 Ram ProMaster 2500 as other commercial operators may face the same sudden dangerous failure.

I started having issues with the power steering. It started when I was leaving work about a month ago. Pulling out of the parking lot,it starting pulling to the left, when I was turning right. I got home and started looking for the power steering fluid tank, and couldnt find it. It was fine for a few days after that. Then I was making a right hand turn, it felt like it starting resisting the turn and tried to straighten out halfway through the turn. Another time, I was going around a left hand curve, maybe 45 mph, it fought back and pulled to the right. I have had a couple other issues, along the same lines over the last month between 5 and 45 mph. I was looking up information on the power steering, finally found it in the owners manual, that I have electric power steering.

Intermittent failure of the electronic power steering

      Incident May 8, 2026

    • Steering, electrical systemFiled May 24, 2026

The vehicle's power steering does not work properly, especially at slow speeds and when attempting to back up. However, it has also failed while traveling on the interstate highway at high speeds. When you attempt to make a turn, the power steering feels like it is fighting you and wants to pull the vehicle back in the opposite direction. I took the vehicle in to be serviced, and they replaced the battery which removed previous warning messages. On two occasions, while traveling at slow speeds and making a turn into a shopping center parking lot, the vehicle resisted my attempt to make the turn such that had a car been coming out of the parking lot, rather than complete the turn, I would have run directly into the side of the other vehicle. While travelling on the interstate at 70 mph, I momentarily lost steering and had there been vehicles behind or beside me at the time, I believe it would have resulted in an accident. Fortunately, the cruise control simultaneously shut off.
